Sometimes called push messaging, directed exchange is the
secure point-to-point electronic delivery of clinical data between
two trading partners who were already exchanging data via fax or
courier (or in some cases, via a web portal). Redwood MedNet helps
our participants replace printed reports previously sent by fax or
portal with a secure electronic delivery service. Making health data
and clinical messages arrive where they are needed in newly installed
EHRs helps simplify patient care workflow and reduces the creation of
new data entry errors. Examples:
- Send laboratory test results as structured data to the ordering
clinician, either via the Redwood MedNet portal or directly into
the clinician's EHR
- Send narrative transcription as electronic text from one health
care site to another (assuming that the receiving EHR is capable
of accepting unsolicited clinical text files)
- Send admitting documents for a long term care resident to a
hospital, or send discharge documents from a hospital to
accompany a patient's admission to a long term care facility
Directed exchange is a key feature of the basic
health data delivery service provided by Redwood MedNet, and
includes a portal for sites with no EHR, or for sites whose EHR
lacks appropriate interfaces for unsolicited electronic clinical
messages. The directed exchange service is especially useful for
hospitals or eligible providers who need to route test results or
care transition summaries to external trading partners.

Community Gateway

Sometimes called query-response messaging, the Redwood
MedNet community exchange gateway provisions two-way messaging
services that integrate with site-level EHR software. For example:
- Order a test from an EHR, and receive the result in the EHR
as structured data
- When a patient presents in the ER retrieve a care summary
from the patient's primary care provider
- Allow a patient's HealthVault
account to query an outpatient practice or hospital for a copy
of a recent visit.
Redwood MedNet can provide this type of bidirectional
gateway service to healthcare facilities with EHR solutions that
are able to support bidirectional exchange services.* The Redwood
MedNet exchange service also includes the
CONNECT federal
gateway, as well as master patient indexing (MPI) services.
Community echange gateway services may include longitudinal patient
records, site or community registries, clinical trials integration,
or other advanced EHR features or community care coordination
options. The success of community exchange gateway services is
dependent upon robust patient consent management at each
participating healthcare facility.
* Not all certified EHRs are able to manage fully informed
patient consent. Even with robust consent management, some EHRs are
not able to support fully bidirectional services. Some EHRs have no
outbound ordering capability, and some EHRs that support outbound
orders have no ability to import an unsolicited clinical result.
Bidirectionality of clinical data traffic is an ad hoc process, and
ranges from easy to prohibitively expensive to impossible.

Population Health

There are three population health data services in development by
Redwood MedNet.
- Push electronic immunization reports from an EHR to the
California Immunization Registry
(CAIR). Launched in August 2011.
- Publish daily syndromic
surveillance signals for influena-like illness to local,
state and national public health agencies. Scheduled for
pilot testing in mid-2012.
- User configurable patient registry builder. In
development. Commercial availability anticipated in 2012.

Patient Portal

Redwood MedNet can exchange* data with a patient's HealthVault
personal health record (PHR) account. Activating HealthVault as a
patient portal is contingent upon affirmative consent and patient
identity authentication. Once activated, HealthVault allows a
patient to organize their own health records, and to control who
sees and uses the records.

Secure HIE Appliance
[ Premium service, extra charge ]
Approximately half of the health care facilities participating in
Redwood MedNet require a hardware
device to secure the HIE communication circuit. There is an extra
cost for installation and maintenance of the secure HIE appliance. An
appliance is typically needed in solo or small practices, or at
independent hospitals.

HIPAA Secure Messaging

Certificate secured SMPT clinical messaging (aka the
"Direct Project") is an
emerging solution for health care trading partners to exchange
patient data. The Direct Project, sponsored by ONC,
establishes standards to support simple scenarios to push data from
where it is to where it's needed in a way that also supports more
sophisticated interoperability in the future. In 2011 Redwood
MedNet received a grant from Cal
eConnect to test secure messaging in production healthcare
environments.
NOTE: In 2012 most certified EHRs will release Direct
Messaging mailboxes that are integrated with the clinician workflow
in the EHR. Redwood MedNet offers a HIPAA secure messaging service
for sites without an updated implementation of a certified EHR, or
for non-standard sites (e.g., long term care, laboratories, etc.)
that do not have a secure Direct Messaging mailbox integrated with
their EHR.

Hosted eRx / EHR

Redwood MedNet has full integration with RxNT,
an ONC-ATCB certified EHR. RxNT can be configured as a simple
stand-alone ePrescribing (eRx) system, or as a full EHR.
